• You a family member want to remain in the home setting but need help to do so. |
•A new caregiver needs instruction and training to provide care for you or a family member. |
Initiating:
• Any individual, physician, or concerned person may request services by calling Green River District Home Health. |
• Following a request for home health care, the patient's physician will be contacted for approval of services. |
• You can initiate services by calling (270) 686-8123 or 1800-928-7123 during office hours: 8:00 AM to 4:30 PM, Monday thru Friday. |
