What you need to know

This service is provided by the Healthy Start in Child Care, through the Green River District Health Department. At this time, this service will be available to only licensed child care centers. You must schedule the visit in advance.

In order to minimize time in the center....

An appointment must be scheduled. Healthy Start in Child Care will mail authorization forms that must be filled out and signed by the child’s parent or guardian. These completed forms will need to be returned to the address below no later than one week prior to the scheduled visit. Healthy Start must have a signed authorization form before the Kid Print ID card can be produced.

The most important information you have concerning your child may only be stored in your head. In the event of an emergency your head may not be as reliable as you think. That is why Amy Brown, Child Care Health Consultant for Healthy Start, has developed this program. It could, one day, be the most important program you allow your child to participate in. Your child's care provider recognizes the importance of the safety of your child and this program. Healthy Start has been invited to visit the center and produce identification cards for the children whose parents allow them to participate. This ID card is about the size of your driver’s license and can easily fit into your purse or wallet.

Please read all the information carefully and make the decision you feel is right for you.
